Supplies needed
Tube(s) of choice, I used Elias Chatzoudis, you must purchase a license found here.
PTU Scrapkit by Shani of Wicked Princess called "Nurse Sugar" and can be found here.
Font of choice
Plugins: Eye Candy 4000, Xero

Please remember to stop and save often.
Add drop shadow of choice as you go. Refer to my example as needed, merge layers down as needed to make things a little eaiser.

Let's Rock this Tutorial!!

Open a new image, 700 x 700 pixels, transparent. You may crop/resize later.

Open paper 7, and copy. Go to new image, Select - All, paste into selection, Select - None.

Open the heartframe, copy and paste as new layer. Resize taking up most of the image, arranging in the middle.

With your magic wand settings at: Mode: Add(shift); Match Mode: None, click in the middle of each heart. Go to Selections - Modify - Expand by3- Selections - Invert. Got to paper layer and hit delete key on keyboard. Select None.

Open tube of choice copy and paste as new layer above the paper layer, resize and arrange inside one of the hearts. Go to the heart frame layer and with your magic wand click inside that heart. Go to Selections - Modify - Expand by3- Selections - Invert. Got to the tube layer and hit delete key on keyboard. Select None. Do this with each heart of the frame.

Go to Layers- View - None. Make each layer of the tube visible, Go to Layers - Merge - Visible. Go to Layers -View - All. Make this layer visible by around 75 percent. Go to Effects - PhotoEffects - Time Machine With Intensity set at 30 using the earliest time, mine is 1839. Go to Effects - Plugins - Xero -Porcelain with these settinggs: Softness: 50; Strength: 128; Brightness: 128; Red Channel: 0; Green Channel: 0; Blue Channel: 255.

Open the square frame from the kit, copy, paste, resize (making if more of a rectangle) and arrange to fit in the heart frame. With your selection tool set to rectangle, draw out a rectangle around the button holes of the frame. Go to Layers- New - Raster layer, arrange this layer under the rectangle frame. Open paper6 copy and paste into selection, Selections - None.

Open broken heart, and statline resize and arrange inside frame towards the top.

Open tube of choice resize and arrange to liking.

Open clipboard resize, copy, paste as new layer above the rectangle frame, arrange to liking. add wording of choice to fit on the paper in the clipboard.

Open first aid kit, ivmachine, marker, pill bottle, misc. pills, brokenheart, RX , resize and arrange to liking.

Open Nurse id paste tube and resize to fit inside the pic part of the id. resize, copy merged paste as new layer and arrange to liking.

Add name , Copyright info, license and save!
